Application Rules

Licensed Shop

Repairs must be done by a licensed and insured repair shop.

We will require a W9 from the repair shop

Owner

The car must be owned by the applicant. 

We require a copies of:

  • Car Registration

  • Drivers License 

Repair Quote

All repairs will need to be quoted from a repair shop.

Maximum approval amount is up to $700

Payments

We will work directly with the repair shop to make paments, we do not pay applicants directly

Application Guidelines

  • We serve the 10 NorthEast Counties of Wi:

Brown, Florence, Forest, Langlade, Marinette, Menominee, Oconto, Oneida, Shawano, and Vilas Counties

  • Car must be used for either work or medical appointments in order to be considered for repair assistance

Zero Income Form

If a household member over the age of 18 doesn't have income, we require a completed zero-income form to be completed and submitted with the application.
